Ref. CS-104 · European passports: EU, EEA, Switzerland

The 790-012 fee form, from blank page to paid receipt

Fill in the Modelo 790, código 012 on the Policía Nacional’s website, tick the one box for your procedure, and pay it online or at a bank before your appointment. It collects the tasa (State fee) of police procedures for foreigners. This page explains the form, not its price: the form shows the current amount once you choose your procedure.

01Where the form is

The Policía Nacional publishes the form online. You fill it in on screen: personal details copied from your passport, the NIE when there is one, and your address. Then you print it, or pay it online straight away. The appointment is booked apart, for free, on the booking website.

02The box that names your procedure

The form lists the procedures in Spanish, each with a small box to tick, the casilla. Tick one box only: the one for the procedure you booked, for example the number alone on form EX-15. Some lines look alike, such as a first card and a renewal, so read each one to the end.

Authorisations decided by the Oficina de Extranjería, such as a work permit, are paid with other codes of the same Modelo 790. The 012 is for police procedures.

03Two ways to pay

  • At a bank that collects State fees: you hand over the printed form, pay in cash, and the bank stamps your copies
  • Online, by card or through your bank, where the form offers it: you then keep the receipt it gives you

04The copy that goes in your folder

The printed form has separate copies: one for you, one for the administration and one for the bank. Bring the stamped copies, or the online receipt, to the appointment. Without proof of payment, the officer cannot go on with your request. The same form, with its own box, also pays for a return authorisation.

05A last check at home

  • Your name is spelled as in your passport
  • Your NIE or passport number has no typing error
  • The box you ticked is the procedure you booked
  • The form shows that it is paid

Questions on this sample

What does it cost?

The form tells you: once you tick your procedure, it shows the amount in force. Amounts change, so this site never prints them.

Does paying the fee give me an appointment?

No. The appointment is booked separately, for free. The fee pays for the procedure itself.

I paid for the wrong procedure. Can I use it anyway?

No, each payment matches one procedure. A refund can be asked for in a separate procedure, so it is worth checking the box twice before you pay.
